A real estate open house sign-in sheet PDF should be plain: printable, readable, and easy to fill out at the door.

I would use the PDF as the paper fallback for QR check-in. It should not try to become a full buyer questionnaire.

Start with one printable page

The PDF should fit cleanly on one letter-size page. Its header carries the property and agent details. Below that, the visitor table needs enough space for handwriting.

If the sheet needs a second page, I would cut fields before I print it.

Fields I would include

For a real estate open house, I want enough information to follow up without slowing down the entry table.

Fields I would skip

I would skip full home address, family details, broad lifestyle questions, and long buyer-preference grids.

HUD's Fair Housing Act overview lists protected categories including race, color, national origin, religion, sex, familial status, and disability. A door sheet should stay focused on the visit, the property, contact details, and follow-up.

Add the property and agent header

The top of the PDF should show the property address, date, agent name, brokerage, phone, and email.

That header makes the sheet usable after the event. It also gives visitors a clear contact point if they take a photo of the sheet or ask for the agent's details.

Pair the PDF with a QR form

I would put the printed PDF beside a QR code. The QR form should be the primary path because it saves clean lead records immediately.

The PDF is still useful when someone does not want to scan, the cell signal is poor, or the open house gets busy.

Print the QR destination

The FTC warns that QR codes can send people to spoofed or harmful sites. A real estate open house QR code should show a recognizable domain or short URL under the code.

That gives visitors a way to check the destination before they scan and a backup path if the camera app fails.

Do not mix it with buyer-agreement paperwork

NAR says visitors who simply attend an open house on their own do not need a written buyer agreement just to tour.

Keep the PDF as a sign-in and follow-up record. Use separate brokerage workflows for representation agreements, disclosures, and required forms.

The follow-up step

After the open house, I would move the PDF entries into the lead list and tag the property, date, agent status, feedback, and next action.

The PDF helps capture names. The follow-up system is where it becomes useful: send the property link, answer questions, respect agent status, and keep the seller activity notes organized.
